ABSTRACT

The paper sorts out and analyzes papers on medical English teaching and research published in core periodicals of China. The results show that there are various problems: slow research progress, insufficient empirical studies, a shortage of research on teachers and students, a lack of young faculty members and insufficient international perspective. With the increasing requirement for medical students' higher language proficiency imposed by the development of higher medical education in China, this paper puts forward some suggestions based on the analysis of the past ten-year data on medical English teaching and research: to build stronger medical English teaching faculty and to enhance their research competence by means of improving, enlarging and cooperating; to intensify the research into the two subjects in the teaching process: medical English teachers and medical students; and to avoid depending too heavily on questionnaire survey and employ multiple research methods and collect effective data in order to fundamentally improve medical English teaching and research.

ABSTRACT

Objective@#To improve the accuracy of diagnosis and to reduce the misdiagnosis rate of nasopharyngeal carcinoma by analyzing the characteristics of such masses. @*Methods@#Clinical data from 55 patients with suspicion of nasopharyngeal carcinoma diagnosed and treated between March 2016 and September 2017 were analyzed. All patients were followed up regularly. @*Results@#With following-up of 12 to 25 months, 6 (10.9%) of 55 cases were identified as nasopharyngeal malignant tumors, including 4 cases of nasopharyngeal carcinoma and 2 cases of lymphoma, and 49 cases (89.1%) were diagnosed with nasopharyngeal benign masses, including 29 (59.2%) cases for nasopharyngeal lymphoid proliferation, 15 (30.6%) for adenoid hypertrophy, 2 (4.1%) for nasopharyngeal cyst, 1 (2.0%) for polyp, 1 for papilloma and 1 for nasopharyngeal pharyngeal cyst. Small nasopharyngeal malignant tumor and masses with benign hyperplasia showed the overlap of images on the enhanced MRI/CT and Fibro-nasopharyngoscopy, but all 6 patients with nasopharyngeal malignant tumors presented with moderately enhanced multiple enlarged lymph nodes. @*Conclusions@#Fibro-nasopharyngoscopy and enhanced MRI/CT have some value on evaluation of nasopharyngeal masses, but biopsy is a golden standard for diagnosis. Follow-up is necessary for the patients with negative biopsy and benign nasopharyngeal hyperplasia indicated by fibro-nasopharyngoscopy and enhanced MRI/CT.

ABSTRACT

Objective To explore the role of homocysteine(Hcy)on angiogenesis at peri infarct region after focal cere-bral ischemia in rats, to elucidate inhibitory factors of angiogenesis, and to establish a clinic foundation for clinical brain functional recovery. Methods Spragur-Dawley (SD) male rats (n=36) were randomly divided into three groups with 12 rats in each group including Sham Operation (SO) group, Middle cerebral artery occlusion (MCAO) group and MCAO+Hcy group. The rats in Sham and MCAO groups were intra-peritoneally injected with 5 mL/(kg·d) saline and rats in MCAO+Hcy group were injected with 2%5 mL/(kg·d) Hey solution from the same route. MCAO was introduced by intraluminal filament meth-od after 7 d Hcy intervention. Rat brains were harvested on the 7th day after MCAO. BrdU(50 mg/kg, as a marker of cell pro-liferation)was intraperitoneally injected three days before the rats were killed. High performance liquid chromatography (HPLC)was used to measure serum Hcy concentration in rats. Brain infarction size was observed by TTC staining. Immuno-fluorescence staining was used to detect the number of BrdU+/laminin+cells at the thalamus of infarction side. Results Se-rum Hcy concentration significantly higher in MCAO+Hcy group than in SO and MCAO groups(P<0.05). Brain damage increased and the number of BrdU+/laminin+cells decreased in MCAO+Hcy group compared with those of MCAO group (P<0.05). Conclusion Increased Hcy concentration in rats lead to more severe damage of cerebral infarction as well as to inhibit the angiogenesis at surrounding ischemia area.

ABSTRACT

Objective To investigate the protective effects of folic acid on the oxidative damage that ox-LDL (oxi?dized low-density lipoprotein receptor 1) render to human umbilical vein endothelial cells (HUVEC). Methods HUVECs were injured by ox-LDL (120 mg/L) for 24 h while they were incubated with various concentration of folic acid (0,15, 60, 150, 225, 300, 375 nmol/L). Then HUVECs were cultured in media contains same concentration of folic acid but without ox-LDL for 72 hours. Finally, HUVECs were harvested after 24, 48, 72 and 96 h. The morphological changes were observed us?ing inverted microscope and cell viability were examined by MTT. Results Various concentrations of folic acid (0,15, 50, 100, 200 and 500 nmol/L) has no obvious promotion or inhibition effect in growth of normal HUVEC (P>0.05). However, compared with the ox-FA-def group, 150, 225, 300 and 375 nmol/L of folic acid promoted proliferation of HUVECs with 96 and 120 hours of incubations (P < 0.05). Folic acid of 60, 150, 225, 300 and 375 nmol/L promoted the proliferation of HUVECs with 72 h and 96 hours of incubation (P<0.05). Conclusion High dose folic acid can reduce the ox-LDL oxida?tive damage on HUVEC in a concentration dependent manner.

ABSTRACT

OBJECTIVETo retrospectively analyze the advantages and disadvantages of radiographic methods commonly used for diagnostic of condylar fractures.

METHODSFrom Jan 2002 to Nov 2013, 290 patients (405 condylars) in the temporomandibular joint (TMJ) division of Ninth People's Hospital Shanghai Jiao Tong University School of Medicine were diagnosed as condylar fractures. Panoramic films and CT were taken in all patients to check and count the amount of condylar fractures, including intracapsular condyle fracture (type A, B, C and M), condylar neck fracture and subcondylar fracture. MRI was also taken in 119 patients with 174 condylar fractures to check the position of TMJ disc. The data were analyzed and compared among the three examinations in the diagnosis of the condylar fractures.

RESULTSPanoramic films showed 79.8% (323/405) condylar fractures. Among condylar fractures, intracapsular condylar fractures, condylar neck fractures and subcondylar fractures accounted for 48.9% (198/405), 20.3% (82/405) and 10.6% (43/405) respectively. CT showed 64.0% (259/405) intracapsular condylar fractures, 24.0% (97/405) condylar neck fractures and 12.1% (49/405) subcondylar fractures. Among intracapsular condylar fractures, Type A fracture was the most common type of ICF, which accounted for 48.7% (126/259), followed by Type B fracture, which accounted for 30.9% (80/259) and Type M fracture, 12.4% (32/259). Type C fracture was the least type which accounted for 8.1% (21/259). According to the diagnostic criteria of CT, there were 10 condylar neck fractures misdiagnosed with intracapsular condylar fractures. MRI showed 94.9% (129/136) TMJ disc displacement in intracapsular condylar fractures, 53.6% (15/28) in condylar neck fractures and 60.0% (6/10) in subcondylar fractures. Among intracapsular condylar fractures, there were 95.3% (61/64) TMJ disc displacement in type A, 95.2% (40/42) in type B, 89.0% (8/9) in type C, and 95.2% (20/21) in type M. There was significant difference of TMJ disc displacement between intracapsular condylar fractures and condylar neck fractures or subcondylar fractures (P < 0.05), and no significant difference between condylar neck fractures and subcondylar fractures (P < 0.05). There was also no significant difference among various type of intracapsular condylar fractures.

CONCLUSIONSPanoramic films can initially diagnose condylar fractures but with the high misdiagnosis rate for intracapsular fractures, minor fractures and fractures without fragment displacement. CT, especially coronal CT, should be considered for positioning fracture lines and confirming the displacement angle of fragment. MRI should also be used to determine the position of TMJ disc in intracapsular condylar fractures.

ABSTRACT

Objective To explore the effects of homocysteine (Hcy) on neural stem cell (NSC) proliferation and the mRNA expression level of Notch1 and Hes1 related Notch signaling pathway from neonatal rats in vitro. Methods NSCs from neonatal rats were cultured by serum-free culture method in vitro. Cells were divided into four groups: control group (Hcy-C), low dose Hcy (Hcy-L) group, middle dose Hcy (Hcy-M) group and high dose Hcy (Hcy-H) group. NSCs were iden- tified by immunofluorescent staining using the antibodies against Nestin, β-tubulin Ⅲ and GFAP. The proliferation ability of NSCs was detected by MTT. The mRNA expressions of Notch1 and Hes1 were detected by Real-time PCR. Results In the serum free suspension medium, neurospheres that consisted of a great number of nestin-positive cells were found. β-tu- bulin Ⅲ positive neurons and GFAP positive astrocytes were detected by immunofluorescence staining on the 6 th day of cell induction. MTT assay showed that the cell viability was significantly lower in three Hcy treatment groups than those of con- trol group (P < 0.05). And the effect of concentration-dependent was observed. The results of RT-PCR showed that mRNA expression of Hes1 was significantly lower in three Hcy treatment groups than that in control group (P < 0.05). The mRNA ex- pression of Notch1 was significantly lower in Hcy-H group than that of other three groups (P < 0.05). The mRNA expression of Notch1 was significantly lower in Hcy-M group than that of Hcy-L group and control group (P < 0.05). Conclusion Hcy could inhibit the proliferation of NSCs by down-regulating mRNA expression levels of Notch1 and Hes1 genes related to Notch signal pathway.

ABSTRACT

Objective To improve the arthroscopic posterior cruciate ligament (PCL) reconstruction using tibial Inlay technique. Methods The special arthroscopic device and related fixation technique were designed. Five cadaveric knees were used to simulate the process of arthroscopic posterior cruciate ligament reconstruction using tibial Inlay technique. The knees were cut open to observe whether the outlet of the tibial tunnel shape and location met the design requirements. Thirty normal MRI films were measured to identify tunnel angle and localizer angle. Results The inner outlet of tunnel was conical shape(14 mm×7 mm×15 mm) and the outer outlet was cylinder-shaped (a diameter of 7 mm). The tibial drill was designed into a split structure and could be assembled in vitro. According to the data obtained from MRI films, the angle between the plane of posterior cruciate ligament and horizontal place was 36°-47°, and the localizer was fixed at 50°.The achilles tendon was used as implant and the allogft bones were designed into conical shape to fit the inner outlet of tunnel. The other end of implant to the proximal tibia was fixed with button plate. All reconstruction operations were performed under arthroscopy. The outcomes of procedure were satisfactory. There were no vascular or peripheral nerve injuries in the cadaveric knees The tunnel position was accurate and the shape of tunnel had met the design requirements. Conclusion Our results imply that improved arthroscopic of posterior cruciate ligament using tibial Inlay technique is simple, accurate, rapid and stable fixation.

ABSTRACT

BACKGROUND: Previous studies have verified that under normal culture of neural stem cells (NSCs), folic acid can accelerate proliferation of NSCs by phosphorylation of mitogen activated protein kinase path activation ERK1/2. OBJECTIVE: To investigate the effect of folic acid on NSC extracellular signal regulatory protein kinase pERK1/2 under hypoxic condition. METHODS: NSCs from Neonatal rats were cultured in vitro by serum-free culture method, and incubated in a flask at 1×10~8/L. Except normal control group, self-made hypoxia equipment was used in the hypoxia model, folic acid deficiency and folic acid supplemented groups at day 3. At 37 ℃, hypoxia culture was conducted in the thermostat for 6 hours. The contents of folic acid were 4 mg/L, 4 mg/L, 0.65 mg/L, 8 mg/L in the four groups. Cells following 6 days were collected to count the density using trypan blue. RT-PCR was utilized to detect pERK1/2 mRNA expression. Western blot assay was employed to determine pERK1/2 protein expression. RESULTS AND CONCLUSION: Compared with the normal control group, the proliferation of NSCs and the expression of ERK1/2mRNA and pERK1/2 protein were decreased significantly in the hypoxia model group. Compared with the hypoxia model group, the proliferation of NSCs and the expression of ERK1/2mRNA and pERK1/2 protein were increased in the folic acid supplemented group, whereas decreased in the folic acid deficiency group. There were significant differences among groups (P < 0.001). Above-described results verified that folic acid supplementation can activate ERK1/2 phosphorylatin and accelerate proliferation of NSCs under hypoxia condition.

ABSTRACT

The intakes of Al, Ca, Mg, Zn, Fe or P in the diet of college students were determined with chemical methods and the effects of Al on several elements were observed. The results showed that the mean intake of Al was 4.59 mg/d for students and the mean daily intakes of Ca, Zn, Fe were 40.1%, 81.8%, 191.1% of RDA, respectively. Higher intake of Al (33.88mg/ person/d) did not apparently influence the serum Al, Ca, Mg, Zn and Fe levels in short time. The multiple stepwise regression analysis found that there was a positive correlation between the serum Al content and P intake and a negative correlation between the serum Al content and Mg intake. Further studies were needed for the effects of Al on Ca, Mg, Zn, Fe and P metabolism.

Objective To investigate the effects of genistein(GEN) on the expression of collagen I and transforming growth factor-?1(TGF-?1 ) of osteoblast.Method The secondary generation of skull osteoblast of newborn SD rat was incubated with GEN.The cells were divided into six groups:control group, different dose of GEN(10-8,10-7,10-6,10-5mol/L,respectively) groups and E2 group( 10-10mol/L).MTT(OD),the contents of cell protein, the activity of alkaline phosphatase(ALP),the expression of collagen I and the content of TGF-?1 were detected. Results After 48h and 72h,the MTT(OD) of all GEN group and E2 group were significantly higher than those in control group. The MTT(OD) of control group and 10-8,10-7,10-6mol/L GEN groups in 72h were significantly higher than those in 48 h.The protein of 10-5,10-6 mol/L GEN group and E2 group were significantly higher than those in control group. The ALP activity of all GEN groups and E2 group were significantly higher than those in control group. The level of above indices were correlated with the dose of GEN.The expression of collagen I and the content of TGF-?1 in 10-7,10-6,10-5mol/L GEN group and E2 groups were higher than those in control group. They werecorrelated with the dose of GEN and TGF-?1.Conclusion GEN could stimulate proliferation and differentiation of osteoblast, and enhance the expression of collagen I and content of TGF?-1.Compared with E2,,there were similar effects with the higher dosage of GEN.

ABSTRACT

Objective To explore the effect of folic acid on neural stem cells(NSCs) proliferation from fetal rats in vitro.Method NSCs were isolated and cultured by microdissection, mechanical blowing and serum-free suspension culture, and identified by immunofluorescent staining using antibody against nestin.BrdU(5’bromo-2’deoxyuridine) was used to mark dividing neural stem cells.Cultured NSCs were divided into four groups:control group, low, high dose group(liquid media with added 4,40 mg/L folic acid),and deficiency group(liquid media with added 0.4 mg/L methotrexate,MTX).Monotetrazolium(MTT) and double-label immunofluorescence technique detected NSCs proliferation under the condition of folic acid. Results In the serum-free suspension medium, neurospheres that consisted of a great number of nestin-positive cells could be obtained. The proliferative ability of NSCs were observed by BrdU labeling methods.MTT assay and double-label immunofluorescence for nestin+BrdU showed that the growth tendency was increased with folate concentration in the medium.Compared with control group,NSCs growth rate of folate group was significantly increased in vitro. Conclusion The culture of NSCs isolated from fetal rats possesses the abilities of proliferation and self-regeneration.Folic acid may stimulate proliferation of NSCs efficiently.
